What is the Edward W.H. Tremain Award of Excellence?

CAMH makes this award to recognize the effort and commitment of staff who offer outstanding service.


Why does CAMH make this award?

Staff excellence helps CAMH reach its goal of improving the lives of people with mental illness and addiction through better understanding, treatment and care.


Who is Edward W.H. Tremain?

Ted TremainEdward (Ted) W.H.Tremain devoted 15 years of volunteer service, first to the Clarke Institute of Psychiatry, then to the Clarke's Foundation and ultimately to the CAMH Foundation. Ted has been a leader in the fight against mental illness and addiction. He has always worked hard, with dignity and humility -- avoiding praise for himself, but quick to ensure that others were praised. This award is named for a man who has made and continues to make a positive difference in the lives of many -- he exemplifies the term "excellence."


Who is eligible for the award?

Anyone who works at CAMH can be nominated. More than one award is given each year.A person can only receive the award once.

How are people nominated?

Anyone can nominate a colleague or supervisor.The nominator must get two other people to support the nomination (three people in total). At least one of these three sponsors must be from a department other than that of the person who is nominated.

How are winners chosen?

The award is granted to staff who are shown to excel in the following:

Performance: Outstanding performance that consistently exceeds goals and job expectations in quality and quantity.

Creativity: Excellent innovation or creativity that is consistent with the vision, mission and goals of CAMH.

Organization: Extraordinary skills in leadership, organization or project management that help CAMH or their department succeed at significant goals and efficiencies.

Teamwork: Superior interactions and co-operation with, and a positive influence on, managers, peers, supervisors, the CAMH community and the client population served. This person will be known as "service-oriented" and a key resource person.This category may also recognize individual contributions to CAMH's or a department's community and diversity efforts.

How do I submit a nomination?

The nomination form should be completed and submitted to:

The nomination deadline is Friday, June 27, 2003, 5:00 pm.

Please fill out the form below or the form that is available on the CAMH Web site. Please provide enough information to help the committee make its decision.We will not accept incomplete forms. If you need help completing this form, please contact Lisa Schmidt at 416 535-8501 x 6663.

All information is confidential and will be reviewed by the Edward W.H.Tremain selection committee.

The awards will be presented at a special ceremony on Wednesday, October 1, 2003.
